Zen Massage Intake Forms are all online.... 

When you book your 1st appointment you will receive a Confirmation Email that will  prompt you to complete your Intake Forms which include Health Information, Conditions, Consent & Policy forms. These forms MUST be filled out online before your 1st appointment. These forms will be resent to you once a year for updates and you will be able to update the forms when needed.

You will also receive an email/text reminder 24 hrs before each scheduled appointment that will prompt you to fill out a COVID-19 Pre-screening Form that must be read and signed within 24 hrs of your appointment.
